The focused setup

    Open the Google Form

    Open the Form you want to monitor in the Google Forms editor.

    Add the Slack Webhook

    Create an Incoming Webhook for the channel that should receive alerts.

    Write a message or payload

    Insert variables from the current Form questions.

    Add a field filter

    Choose equals, contains, or greater than, then test the latest response.
